Acknowledgement of Services and Assumption of Risk Agreement

ACKNOWLEDGEMENT OF EDUCATIONAL SERVICES

I understand and agree that all communications, quizzes, video messages, digital materials, and exercise programs provided by Brandon Launstein, DPT, and Lumbar Performance Institute prior to a formal, live video clinical assessment are intended strictly for general educational and informational purposes.

I explicitly agree that these interaction(s) do not constitute medical advice, physical therapy treatment, or a formal diagnosis. Interacting with this page, submitting a quiz about my symptoms, or receiving an automated or video screening response does not establish a formal physical therapist-patient relationship as defined by state or federal law. This content is not a substitute for an individualized evaluation or treatment plan by a licensed medical professional.

ASSUMPTION OF RISK

I acknowledge that physical exercise carries inherent risks of discomfort or injury, particularly if performed incorrectly or applied to an unassessed physical condition. If I choose to perform any movement, drill, or exercise described on this page or in communications with Brandon Launstein, DPT, I do so voluntarily and entirely at my own risk.

I accept full responsibility for determining whether I am physically safe to perform any suggested movement. If an exercise requires assistance from a family member or third party ("Helper"), I assume all risks associated with the actions or assistance of that Helper. I understand that I should schedule a formal telehealth or in-person evaluation with a licensed physical therapist or physician for personalized medical direction.

WAIVER OF LIABILITY

I expressly agree that this agreement is intended to be as broad and inclusive as permitted by the laws of my state of residence. By accessing this page, submitting a quiz about my symptoms, or engaging in communication with Brandon Launstein, DPT, or Lumbar Performance Institute, I knowingly and voluntarily forfeit any right to bring legal action or assert a claim against Brandon Launstein, DPT, Lumbar Performance Institute, or its representatives for any injury, loss, or damage arising from my participation in these self-directed educational programs.
